Vessel Operation
- All Vessels through their respective agents should submit Declaration of Medical Health prior arriving at Port. This form shall be filled and submitted as part of pre-arrival questionnaire 72-48 and 24 hours prior to vessels arrival.
- Any vessel visited any ports of the high alert countries in the last 30 days should submit the Coronavirus declaration along with the maritime health declaration at least 48 hours prior to arrival to any port of State of Qatar.
- This document will be submitted to concern authorities for further verification and approvals for vessel berthing.
- Vessels calling directly from Iran will be allowed to berth only after approvals from authorities.

- Last ten Ports of call
- Crew list with crew joining date and place
Crew Change and Related Matters
- Crew change is possible, subject to pre-approval.
- For any crew change, please contact your respective agent well in advance and notify your plan.
- All vessels visiting Hamad Port should complete COVID-19 Health Declaration before 72 hours of arrival.
- Emergency medical is permitted, in such cases crew will be taken to hospital by ambulance. All medical emergency, Master should report to Port Tower by calling on VHF and inform the agent simultaneously.
- No shore leave permitted
Ship Supplies
- Ship supplies including provision, spares, freshwater etc are possible however the master should inform the requirement well in advance ( at least 72 hours before), so arrangements and approvals can be made accordingly.
